| 9:30 am – 10:30 am | Compassionate conversations, Compassionate documentation Pregnancy & Infant Loss Network When a family experiences pregnancy and infant loss, they will often look to their healthcare provider for guidance, support, and information. These non clinical interactions could include important details about the loss, assessments of the family’s emotional state, and information about grief supports or other referrals made. These conversations can be difficult for providers to have, and difficult for families to remember. In this session, we will discuss how to document these conversations and interactions in a sensitive way that would be helpful for a family to read at a later date should they access their health records, and inform process improvement as it pertains to bereavement care and support. | 11:00 am – 12:00 pm | How Patient Ombudsman uses data (its own and other sources) to identify trends in complaints and make decisions to initiate investigations. Trend analysis is part of the role of an ombuds – to shine a light on emerging issues and serve as a “canary in the coal mine”. This presentation would look at how we use our own data as well as other sources to identify trends in complaints to spot patterns, systemic issues, and make decisions to initiate own motion investigations. With a strong background in health information systems and data analytics, he began his career as a co-op student at the Canadian Institute for Health Information (CIHI) before transitioning to an analyst role. Ash expanded his expertise at the National Research Corporation (NRC), collaborating with hospitals across Canada, and later at Michael Garron Hospital, where he has made significant contributions. He holds a post-graduate certificate from George Brown College and an executive Master of Health Informatics from the University of Toronto’s Institute of Health Policy, Management, and Evaluation (IHPME). |
