This presentation highlights effective collaborations and partnerships among health care professionals in pursuit of quality improvement. Learn from seven speakers as they discuss patient quality, safety outcomes and clinical documentation improvement (CDI) results at Surrey Memorial Hospital – part of Fraser Health Authority in British Columbia.

This journey is facilitated by a physician quality lead and a clinical documentation improvement specialist lead, and the HIM program coordinator at Douglas College. Students enrolled in the second year of Douglas College’s HIM program are also featured and given the opportunity to share their learnings from this initiative.

Hanifa Ladhani, CHIM, CCDIS Clinical Documentation Improvement Specialist, Coding and Informatics Services, Health Information Management, Fraser Health

Hanifa Ladhani has a diverse background that spans over 25 years working as a coder, analyst, systems support, and electronic health record trainer in British Columbia across Lower Mainland Health Information Management, including Fraser Health Authority (FHA), Providence Health Care (PHC), Provincial Health Services Authority (PHSA), and Vancouver Coastal Health (VCH). This year, she is celebrating ten years working as a Clinical Documentation Improvement Specialist.

At Fraser Health, she is an active committee member for Patient Safety Priorities and Sensitive Adverse Events, utilizing her knowledge and experiences to accurately document patients’ clinical journeys for continuity of care and coding. She is passionate about engaging and supporting stakeholders and clinical and coding teams to improve patient quality and safety outcomes.

Hanifa assisted with developing the Canadian College of Health Information Management’s Certified Clinical Documentation Improvement Specialist (CCDIS) national certification examination. She is also a committee member for the Canadian Health Information Management Association (CHIMA) British Columbia and Yukon Chapter.

Marietta Van Den Berg, MMed Psychiatry Physician Quality Improvement Lead, Surrey Memorial Hospital

Marietta Van Den Berg is a general adult psychiatrist and currently the physician QI lead in the urgent care response center for Surrey hospitals in British Columbia.

Marietta is interested in urgent/emergent psychiatry and borderline personality disorders and is part of the Dialectical Behavioural Therapy (DBT) team in Surrey. She qualified as a physician in 1988 and toured through orthopedics, rehabilitation medicine, palliative medicine, infectious diseases, and military medicine before becoming a psychiatrist. She has loved the opportunities for system redesign that the high-pressure, high-volume patient load in Surrey necessitates.

As an immigrant to Canada, Marietta’s third country, she feels lucky to live in beautiful, green, and rainy British Columbia!

Pat Visosky, MSc, PhD, CHIM HIM Program Coordinator, Douglas College

Pat Visosky has been the coordinator of the health information management program at Douglas College since 2016. Before that, Pat was faculty of the bachelor program in HIM at the Higher Colleges of Technology in the United Arab Emirates for 17 years. Pat is a certified HIM professional and has a keen interest in health informatics. Over the years, she has obtained her Master of Science in health informatics and recently completed doctoral studies in health research focusing on the health informatics curriculum.

Ann Dreolini, MLIS

Ann Dreolini is a former librarian and medical office assistant. She is currently a student at Douglas College in the Health Information Management program. 

Elena Zamedyanskaya, PBD-HIM

Elena Zamedyanskaya graduated from Moscow State University of Culture and Arts in 2008, majoring in socio-cultural activity and specializing in tourism and hospitality management. She spent nearly a decade working for travel companies organizing trips and children’s camps. Elena also worked for an immigration and visa services office for four years. In May 2021, she graduated from the Douglas College Health Information Management program and earned a Post-Baccalaureate Diploma.  

Justina Bliss, MASc

Justina Yang Bliss is a brand-new 2021 graduate of the HIM program at Douglas College. After working as a consulting engineer and a freelance artist for many years, she is excited to make a career transition into the health care sector and begin her job as a coder at St. Paul’s Hospital in Vancouver, BC. Justina previously earned a master’s degree in structural engineering from the University of Waterloo.
